One of those times that I just draw and add the narrative as I go along and refine it.
I'd imagine that Leon is just outside of a waiting room at a hospital. The smoke doesn't bother anyone but it gives him something to do. Its been a long day. And Leon never looks forward to breaking the news to someone. Especially someone that has actually been improving overall. But sudden downturns happen all the time.
