Dr. Big McIntosh, grandson of Dr. Granny Smith, older brother of Dr. Applejack and Apple Bloom, anesthesiologist at Ponyville General Hospital, dressed in a violet EquestriMed surgical gown, light-violet EquestriMed surgical gloves, a light-blue EquestriMed surgical tail cover which covered his tail, a light violet EquestriMed surgical cap which covered his ears and the most of his hair except for the back, and is breathing in his white EquestriTech EMDBN95-M/L-W duckbill-style disposable particulate surgical respirator mask over his nose and mouth, to protect his lungs from any harmful airbourne particles and nasty germs. Big McIntosh's mask also keeps her muzzle warm.
