It looks like some of the above bugs go away when I turn off ad-blocking, so hopefully that helps others.

There seem to be a few bugs with the Jotform application form. When choosing "don't publish until..." there is no date picker to choose a date. The "number of characters" field is mostly unreadable. There's an orphaned checkbox called "* REQUIRED" that only shows its label when hovered over. Also, I'm not sure why the full legal name is required?
