It really depends on the artist but mostly from what i've seen, most people seem to start of with shapes in some fashion. After some time you don't always need it (yay muscle memory) but i can tell you that i still work from shapes regularily. Building up a strong base really helps the final image imo.

It always feels like telling people that heads/bodies are just shapes is the hardest thing to do. Even when you get to be a "great artist", I'm sure most of those types still rely on figuring out the shapes for heads and bodies of characters--no matter what kind of character it is. :) People seem to look for fast and simple ways to draw, but I think at least sticking to the basics works better in the long run.
