And yes, Donna is disguised as a doctor here. At the beginning, I though about giving her a nurse outfit so that both of them was nurses, but the doctor one seems much more fun in my opinion.
I got the doctor outfit idea after playing through Ratchet and Clank: A Crack In Time in the scene where Qwark, as nurse Shannon, tells the robots that Ratchet is a doctor.

So pretty much have Qwark somehow got Donna into this whole mess, which she's not very comfortable with.
