People who were martyred back in the days of early Christianity were often killed by being nailed to a cross and then the cross was turned upside down. When one is martyred (like Jesus) on a right-side-up cross then you die by suffocation. (Your body has to hold itself up in order to maintain airflow to your lungs and eventually your muscles get too worn out.) However on the upside-down cross you can't suffocate so the death is much slower and more painful as you die through starvation.

I was always under the impression that only St. Peter was martyred that way, by request ("I'm not even worthy to die like Him"), hence why the inverted cross is sometimes referred to as the St. Peter's Cross and adorns the throne of the Pope. I could be totally ignorant if there were others, though; I'm no expert on martyrs or saints! Still sounds like a highly unpleasant way to go regardless.
