Yes! This is Extriverse Unicrons 'true' face! So, while Unicron started out looking like his G1 self initially, he began to change quite rapidly.

The more he ate the more he added onto his own mass, the bigger and bigger he got, he practically has his own ecosystem inside him he's so vast.
Unicron is a cosmic abomination of a god who should never have been able to cross into the mortal realm but he did. His body is hideously distorted, constantly throbbing, shifting and bulging, has metal growing out in awkward places, mouths all over him as he constantly hungers to devour creation and webbed/veined wings.
After all, Primus is asleep and cannot stop him.

I have a sketch of his body, but honestly, I kinda prefer the idea that this heinous entity is always obscured by something preventing him from being fully seen.

Now you understand why many people merely dropped down and wept upon seeing his grisly visage.
