Pretty sure everyone on here has posted this at some point but, uh! Here goes...
I hate turning people down. It agonizes me to say no to someone who just wants to reach out in a friendly way. But I also want the status to have real meaning behind it, and it won't if I accept every person who asks. Just remember that:
"Friend" status is different than "Follow"
+Follow someone if you like their art/content and want to keep up with it.
+Friend someone if you already know them personally and want to mutually acknowledge this fact in public.
I think a lot of people are mistaking sending a +Friend request with the desire to befriend someone. Which it is not...
If you want to exchange Friend status, you gotta do more than just click a button. Do things like... Comment and fave on my pictures and journals. Ask questions about my characters. Leave shouts on my page. Interact with me on Twitter. Do a piece of fan art. Chat with me on IM. Obnoxiously plead for roleplay and free art (just kidding on that one!). Become friends of my friends. Talk to me in a stream. You know, stuff that friends might do.
Yeah I've had one or two like that previously, and people get confused by the +Follow / +Friend thing. But like you I wouldn't add people with +Friend if I didn't want to have them as a friend instead as regular following, unless they're genuinely want to become a friend, or if I've previously known and befriended them before else where.

I think people add the friend status to also get the "can I see your secret porn stash you only show to friends" lol. I only friend people I'm familiar with and will share things that I may not post for regular followers. Those cases will be rare though.
